Required Student Forms

Make sure to read this entire page to find out exactly WHAT forms you need to fill out ahead of time, and bring with you to camp. Students arriving to camp without forms will not be able to register until they can produce them. Form One: Student Medical Information

With so many students in one spot, it is important that our camp staff is able to keep accurate records on the medical and health issues of the students. We will have licensed medical staff on hand, as well as the utilization of the Albion area medical facilities. Please review this form and complete it as honestly and thorough as possible. Form Two: Participant Commitment and Expectations Form

At MASC/MAHS, we hold high expectations for our workshop participants--we know you are leaders in your communities and schools!
