The MAHS Exceptional Service Project Award is an opportunity for all of our member schools to showcase a service project they have completed. ANY service project, big, small, complex, simple– any service project can be submitted! The project does not have to be original, unique, or special in any way. The committee is looking for well-organized projects that challenge the chapter and serve the community in some way. Schools that submit high-quality applications will be awarded with the Exceptional Service Project Award at the MASC/MAHS State Conference. Each year, up to ten (10) schools are awarded!

Examples of some types of service projects that have been submitted in the past:

Chapters must include each of the four (4) sections below. Incomplete or missing sections will result in a disqualification.

The service project the chapter focuses on must take place no earlier than April of the PREVIOUS school year and must be fully completed by the time this Application is submitted THIS school year.

Schools cannot use the same service project if they have submitted it for an Award in the past.

Awards must be submitted in a 3-ring binder (1" maximum). Dividers should be used before EACH of the four (4) sections listed below. DO NOT permanently bind pages together, DO NOT use sheet protectors (unless it is needed for pages with pictures, advertisements, etc...), and DO NOT staple pages together. The committee may need to pull apart the application sections and sheet protectors, permanent bindings and staples make that very difficult!

Of the four (4) main sections, only Section 3 has sub-sections, clearly noted below. Each of these sub-sections should start on a NEW page within that tabbed section. Each new page should be titled to match the sub-section titles given below.

 

SECTION ONE (1): CHAPTER INFORMATION
Please include all of the details outlined below on one concise sheet, typed, double-spaced.

  • School Name
  • Chapter/Charter Name
  • Address
  • City, State Zip
  • Phone Number
  • Principal(s) Name(s)
  • Adviser (s) Name(s)
  • Chapter President's Name

SECTION TWO (2): PROJECT DESCRIPTION
Please include all of the details outlined below on one concise sheet, typed, double-spaced.

  • Service project name
  • Date(s) that the project took place (actual event; do not include planning days)
  • Description of project (overall, general explanation of the service project)

SECTION THREE (3): PROJECT EXPLANATION
Please include all of the details outlined below using a new page for each sub-section (and no more than one page per sub-section), double-spaced. Appropriately title each page (use the sub-section titles below: Goal Setting, Timeline, etc...) and the information for each sub-section should be no longer than one page, double-spaced.

  • Goal Setting: state the goal for the service project (you must use the SMART Goals format)
  • Chapter, School & Community Involvement
  • Organization & Activities
  • Timeline
  • Evaluation
  1. Goal Setting: Using the SMART format, please write the goal you hoped to achieve upon completion of your project. (you must follow the SMART format when writing your goal). The description of your project is noted above, so please do not simply rephrase your project description. Be sure to write your goal in SMART format.
  2. Chapter, School and Community Involvement: Indicate the amount of time or effort involved in this project from NHS members, school faculty, other students or school groups, individuals in the community and which groups were involved. (Parents, church members, etc.)
  3. Organization and Activities: Please provide us with at least three (3) activities/steps you took to achieve your goal, explaining what the activities/steps were, who was responsible, how they accomplished the task, supplies needed, due dates, etc… Be specific enough so that if we wanted to reproduce this event, we would have all of the information we need in this packet.
  4. Timeline: What was the schedule of project development/ implementation / evaluation used by your NHS chapter to reach the goal stated above? Please include the timeline and due dates used.
  5. Evaluation: To what degree was the project a success? Did you reach your goal? How did you evaluate? Please clearly identify the strengths and weaknesses of your project. What did you learn from the completion of this project?

SECTION FOUR (4): VISUAL EVIDENCE
Please provide us with copies or originals of visual evidence supporting your project, such as advertisements, pictures, video tapes, news articles, announcements, publicity samples, letters from those that were on the receiving end of your service, etc...be creative!!
