Behavior and Medical Forms
Every participant (student and adult) is required to bring with them a completed medical/agreement form to conference check in. We have streamlined the forms together, to make it easier for you to complete, and less likely that someone will leave a piece behind. There are separate forms for students and advisers.
Additionally, students are required to complete the medical information section (part one) of the form, and have the entire document verified and signed by a parent or guardian. Medical information is included in the adviser packet, and is highly suggested to complete, in case of an emergency on site. We will not be able to grant you admission to the conference without the completed forms at check in. Advisers or students caught replicating information or signatures will not be granted admission and building administrators will be contacted. NOTE: Advisers/chaperones check for accuracy and completion, but do not collect forms, as each student will need to complete the check in process on their own.
